What To Expect at Your Appointment
At Thrive Pediatrics we value your time and want to maximize your experience with our providers and reduce your in-office waiting time. If you have been here before, please log in to the Patient Portal prior to your visit to fill out your child’s developmental questionnaire and update any medical history. The Healow app allows contactless check-in from your car. Your child’s health is our number one priority, that’s why we designed our office to have separate well and sick areas, including waiting rooms, exam rooms, and restrooms to ensure your health and safety at all times. We follow the American Academy of Pediatrics recommendations for Preventative Pediatric Healthcare.
New Patient Forms
If you are a new patient or will be bringing a new child to visit us, we will need new patient forms filled out to add to your medical files. For your convenience, you can download a PDF of the form and fill them out prior to your appointment. If you are unable to fill out the forms before your appointment, please arrive 15 minutes before to your appointment to complete the forms. Please bring a photo ID to your scheduled appointment and be sure to view our insurance and billing page to see a list of our in-network insurance providers. We look forward to seeing you!
